SCG Awarded Comprehensive End-Stage Renal Disease Care model as part of CMS Quality Measures Assessment Tool Contract

The Centers for Medicare and Medicaid Innovation, under the Centers for Medicare and Medicaid Services (CMS), awarded the system management and integration of the Comprehensive End-Stage Renal Disease Care (CEC) model into the Quality Measure Assessment Tool (QMAT) as a prime contract to Maryland-based Signature Consulting Group(SCG).

The CEC model will be integrated into the SCG-built QMAT for claims and quality measure data collection, data exchange, performance scoring, and reporting. QMAT is an essential system for collection and performance reporting on clinical quality measure data for ESRD Seamless Care Organizations (ESCOs) or Accountable Care Organizations (ACOs).

The CEC model tests an approach for improving the clinical outcomes and reducing the Medicare expenditures for beneficiaries with End-Stage Renal Disease (ESRD). ESRD patients require dialysis or transplantation in order to survive as the kidneys are no longer able to perform life sustaining physiological functions. These patients also tend to have multiple comorbidities, which results in increased complexity of their care. Overall, ESRD beneficiaries are among the most medically fragile and high-cost populations served by the Medicare program.
